Hospice Patient Care Coordinator
This is an office position in charge of scheduling / managing the day to day operations.
The PCC is responsible for the day to day management of patient care activities and supervision of clinical staff. May have to do patient care if needed. The PCC serves as a consultant to hospice staff. Participates in quality improvement program and the evaluation of nursing and other clinical services.
Must have Hospice PCC experience or something similar. Must have 2 years staff nurse experience in an acute care hospital or home health agency. Should have some experience with dying patients and their families with a good understanding of pain / sympton management. Must have 1 year of supervisoty experience.
